Please choose a time slot and follow the instructions to book your appointment for the COVID-19 vaccine.
Be sure to bring your state issued ID/drivers license and your **prescription** insurance card. Please make sure "RxBIN, RxPCN, RxGrp" are on the card, otherwise it is likely to be your medical card that cannot be used in the pharmacy. If you do not have insurance, you will still receive your vaccine as long as you are eligible, more info will be collected for the governmental funding to cover for you at the appointment.
If you are over the age of 65, or who is eligible for the Medicare, please bring your Medicare Part A/B card. This is the Federal issued red, white and blue card. All medicare beneficiaries have one. If you are under "Medicare Advantage" and was told there is no Medicare card, please call 1-800-Medicare (1-800-633-4227) to request a new card, ask for your "MBI" (Medicare Beneficiary Identifier), and bring your MBI to the appointment.
